Common CPT Codes for OCD Treatment
- 90837 (60-minute psychotherapy) is the primary code for ERP sessions. Most ERP sessions run 50-60 minutes.
- 90837 with add-on 90785 (interactive complexity) when the ERP session involves in-vivo exposures requiring additional clinical management or when a family member participates in the exposure exercise.
- 90834 (45-minute) for maintenance sessions after the intensive ERP phase.
- 99214 (E/M) for SSRI or clomipramine medication management visits.
Authorization and Payer Rules
ERP therapy often requires 12 to 20 sessions at 2x per week. Request authorization for the full course upfront. UnitedHealthcare approves 12-session blocks with renewal at clinical review. Anthem covers ERP under standard behavioral health benefits. Aetna requires a treatment plan demonstrating ERP methodology. Common Questions About OCD Billing
Is ERP therapy billed differently from standard CBT?
No. ERP is billed using the same psychotherapy codes (90834, 90837) as CBT. The treatment modality is documented in the chart note, but the CPT code is the same. Payers do not have a separate code for ERP.
Can I bill for extended ERP sessions (90+ minutes)?
Bill 90837 for the first 53+ minutes. For sessions exceeding 90 minutes, add prolonged service code 99354 (first additional 30 minutes) if your payer accepts it. Many payers do not reimburse prolonged therapy services — verify before billing.
