What Is CPT 90785?

CPT 90785 is an add-on code that reports interactive complexity during a psychiatric evaluation or psychotherapy session. It applies when communication barriers or complicating factors require additional clinical effort. The code adds $15 to $35 to the base session reimbursement. It cannot be billed alone — it must accompany a primary therapy code (90834, 90837, 90847, or 90791).

When to Use 90785

  1. Third-party involvement: A parent, guardian, or legal representative participates in the session and requires separate communication management (distinct from family therapy).
  2. Communication barriers: The patient uses an interpreter, has a cognitive disability affecting communication, or requires sensory accommodations.
  3. Emotional complexity: Maladaptive communication patterns (e.g., severe resistance, active psychosis during session) that require the provider to adapt the therapeutic approach mid-session.
  4. Legal or custody involvement: Court-ordered treatment, forensic evaluations, or sessions where documentation serves dual clinical and legal purposes.

Documentation Requirements

The chart note must specify which qualifying factor triggered 90785 — not just that the session was "complex." Payers reject claims where the note says "interactive complexity present" without naming the specific barrier. Document the factor, how it affected the session, and the additional clinical effort required. Pair with the base code on the same claim line.

Does 90785 work with group therapy (90853)?

No. CPT 90785 is approved as an add-on to individual therapy codes (90834, 90837), family therapy (90846, 90847), and psychiatric evaluation (90791, 90792). It does not apply to group sessions.
