BCBSA published a data analysis showing AI-powered clinical documentation and coding tools are driving up hospital bills. Researchers estimate ~$663M in inpatient and $1.67B+ in outpatient spending tied to AI-enabled upcoding. At one facility, case complexity ratings jumped 6.7% after AI adoption vs. 0.9% at peer hospitals in the same state. Blood transfusion rates rose only 0.7–1.1% while posthemorrhagic anemia coding rose 3–15% — a disconnect between billing and treatment. 46% of hospitals now use AI in billing/coding (up 25 percentage points YoY).

IKS Health won three 2026 Black Book Research awards: AI-driven RCM (2nd year), medical coding (4th year), clinical documentation/AI services (13th year). The survey data: 35–55% of providers expect to increase AI-RCM spending in 2026, rising to 45–70% in 2027. Separately, Ensemble's clients received 13 HFMA MAP Awards for revenue cycle performance, reinforcing the trend that AI-augmented RCM is becoming table stakes.

Initial denial rates climbed to 11.81% in 2024 (third consecutive annual increase). Medicare Advantage plans now deny at 17%. Oscar Health leads major insurers at 25.3%, Molina at 22%. Hospitals spent $19.7B fighting denied claims. Nearly 1 in 5 ACA marketplace claims denied (~8.8M of 46M in-network claims). Private payer denial rates average 15% across 516 surveyed hospitals.
