BCBSA released the first large-scale evidence that AI ambient documentation tools are inflating hospital billing. Researchers analyzed anonymized commercial inpatient claims (April 2022 - March 2025) across BCBS plans and estimate $663M in inpatient and $1.67B in outpatient spending tied to AI-enabled upcoding. AI-powered upcoding accounted for roughly 1.8% of total claims increases. One finding: acute posthemorrhagic anemia coding increases at analyzed hospitals added $22M to maternity admission costs in a single year. The top 10% of hospitals drove most of the detected increases.

NYC-based Translucent closed a $27M Series A led by GV (Google Ventures) with NEA, Virtue, and FPV Ventures participating. The company builds an agentic AI platform for hospital finance that unifies clinical, operational, and financial data to detect margin leakage in real time. One customer (Wray Community District Hospital) says Translucent replaced 40-60 hours of spreadsheet work with 2-minute automated reports. Seed round was $7M in August 2025.

CMS published a correction to the CY2026 Physician Fee Schedule final rule today, updating Practice Expense RVUs for several CPT codes based on final OPPS APC payment rates. Separately, multiple industry sources now project commercial/MA denial rates will reach 15-17% in 2026, up from 13-15% in 2025. Experian Health's 2025 State of Claims found 41% of providers face denial rates at or above 10%, with 54% reporting denial rates are increasing. Only 14% of providers currently use AI for claims management — despite 67% believing AI could improve the process.
